We are seeking a dependable, compassionate, and experienced Licensed Practical Nurse (LPN) to provide high-quality in-home nursing care to patients throughout Boca Raton and the surrounding communities. This is an excellent opportunity for an LPN who enjoys working independently while making a meaningful difference in patients' lives.
Responsibilities:
- Provide skilled nursing care in patients' homes under the supervision of a Registered Nurse.
- Perform patient assessments and document clinical findings accurately.
- Administer medications and treatments as ordered by the physician.
- Monitor and report changes in patients' conditions.
- Educate patients and family members on disease management and care plans.
- Maintain timely, accurate, and compliant clinical documentation.
- Communicate effectively with physicians, case managers, and the interdisciplinary care team.
Required Qualifications:
- Current Florida Licensed Practical Nurse (LPN) license is in good standing.
- Minimum of one year of nursing experience (home health experience preferred).
- Current CPR certification.
- Valid Florida driver's license, reliable transportation, and current auto insurance.
- Strong clinical, organizational, and communication skills.
- Ability to work independently and manage a flexible daily schedule.
- Experience with electronic medical records is a plus.
- Must be proficient in the understanding and performance of wound care.
