Full-Time Field LPN

We are seeking a dependable, compassionate, and experienced Licensed Practical Nurse (LPN) to provide high-quality in-home nursing care to patients throughout Boca Raton and the surrounding communities. This is an excellent opportunity for an LPN who enjoys working independently while making a meaningful difference in patients' lives.

Responsibilities:

• Provide skilled nursing care in patients' homes under the supervision of a Registered Nurse.
• Perform patient assessments and document clinical findings accurately.
• Administer medications and treatments as ordered by the physician.
• Monitor and report changes in patients' conditions.
• Educate patients and family members on disease management and care plans.
• Maintain timely, accurate, and compliant clinical documentation.
• Communicate effectively with physicians, case managers, and the interdisciplinary care team.

Required Qualifications:

• Current Florida Licensed Practical Nurse (LPN) license is in good standing.
• Minimum of one year of nursing experience (home health experience preferred).
• Current CPR certification.
• Valid Florida driver's license, reliable transportation, and current auto insurance.
• Strong clinical, organizational, and communication skills.
• Ability to work independently and manage a flexible daily schedule.
• Experience with electronic medical records is a plus.
• Must be proficient in the understanding and performance of wound care.
