Compensation & Schedule

• Up to $45.92 per hour
• Monday–Friday schedule (5x8s)
• No weekends required

Position Overview
We are seeking a compassionate and skilled Physical Therapist to deliver high-quality care to patients in their homes. This role focuses on improving mobility, managing pain, and enhancing overall quality of life. The PT will collaborate with an interdisciplinary care team to create and implement individualized treatment plans tailored to each patient’s unique needs.

Key Responsibilities

• Evaluate and assess patient conditions to develop customized treatment plans
• Provide therapeutic interventions to improve strength, mobility, and function
• Educate patients and families on exercise programs, safety, and long-term care strategies
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate care
• Maintain accurate, timely, and compliant documentation
• Monitor patient progress and adjust treatment plans as needed

Education & Certification Requirements

• Active Physical Therapist license in Iowa (or compact eligibility)
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification preferred

Qualifications

• Experience in home health, hospice, or similar settings preferred
• Strong interpersonal and communication skills
• Ability to work independently in a home-based environment
• Commitment to compassionate, patient-centered care
