Job Title: Behavioral Health Care Manager (Field-Based)

Payrate: $40-$43 an hr
Location: On-site/In-field – Northwest Ohio (Lucas, Fulton, Wood, Ottawa Counties)
Job Type: W2 Contract (Temporary) | Full-Time (40 hrs/week)
Contract Length: ~52 weeks (estimated June 2, 2026 – June 1, 2027 )

Job Summary

We’re hiring a Behavioral Health Care Manager to provide care management services for members, including assessment, care planning, coordination of services, and ongoing follow-up. This is a field-based role supporting members across multiple counties in Northwest Ohio. You must be comfortable working independently, communicating clearly, and adapting to changing priorities while partnering with a larger care team.

Key Responsibilities

  • Provide care management services in accordance with best practices
  • Assess member needs and develop individualized care plans
  • Coordinate with providers, care teams, and family/support systems to support treatment goals
  • Monitor progress and update care plans as needed
  • Educate members and families on available resources and support services
  • Document interventions and communicate updates to appropriate stakeholders
  • Required Qualifications

    • Education (one of the following):
      • Master’s degree in Behavioral Health or Social WorkOR
      • Degree from an Accredited School of Nursing
    • Active Ohio license (required – OH is not a compact state):
      LCSW, LMSW, LMFT, LMHC, or LPC
    • Behavioral Health experience is required
    • 2–4 years of related experience, such as:
      • Behavioral health care management / case management
      • Nursing (case management), home health, behavioral health setting
    • Reliable vehicle and ability to travel weekly within the assigned territory
    • Ability to work independently in a field-based environment with strong time management

    Preferred Qualifications

    • 2–4 years of Behavioral Health–specific experience

    • Flexible, adaptable, open to change, and able to collaborate effectively on a team

    Disqualifiers (Please Note)

    • Inability to work independently
    • Difficulty managing change/fast-moving priorities
    • Candidates seeking long-term permanent roles only (this is a contract position)
