Role Summary

As a Field-Based Case Manager. You’ll play a vital role in responding to non-violent crisis situations and connecting individuals to life-stabilizing resources.

Working as part of a multidisciplinary response team, you’ll be dispatched to real-time calls involving individuals experiencing mental health challenges, substance use concerns, interpersonal conflict, or other non-emergency crises. This is a unique opportunity to be on the front lines of change, providing support, dignity, and hope to those in need.

Key Duties & Responsibilities

  • Respond to non-violent, field-based crisis calls involving behavioral health, substance use, welfare checks, and verbal disputes.

  • Provide on-site de-escalation, crisis assessment, stabilization, and emotional support.

  • Deliver person-centered, trauma-informed care with empathy and professionalism.

  • Link individuals to essential services, including housing, medical care, mental health treatment, and substance use recovery programs.

  • Provide immediate and follow-up support to ensure successful engagement with community resources.

  • Document all interactions and services provided in alignment with program protocols and best practices.

  • Work closely with fellow crisis responders, supervisors, and community-based partners to ensure coordinated and effective service delivery.

  • Attend and actively participate in case reviews, team meetings, and ongoing training.

  • Maintain strict confidentiality and comply with all ethical and legal reporting requirements.

  • Fulfill other responsibilities as assigned to support program operations and mission impact.

Required Qualifications

  • High school diploma or GED.

  • At least 1 year of experience in case management or street outreach, preferably with unhoused or vulnerable populations.

  • Valid California driver’s license, clean driving record (past 3 years), and eligibility for coverage under agency insurance.

  • Ability to pass a comprehensive background check, including Live Scan fingerprinting.

Desired Qualifications

  • Bachelor’s degree in Social Work, Psychology, or a related human services field.

  • Experience supporting individuals with mental health or substance use challenges.

  • Bilingual in Spanish and English.

  • Training or experience in crisis intervention, de-escalation techniques, or harm reduction models.

Work Schedule & Setting

  • Full-time position; 12-hour shifts (9:00 AM – 9:30 PM or 9:00 PM – 9:30 AM).

  • Rotating schedule: Three days on, followed by four days on, including weekdays, weekends, and holidays.

  • No on-call duties required outside of scheduled shifts.

Work Environment & Physical Requirements

  • Field-based role working in diverse community settings across the Los Angeles service area.

  • Ability to sit, walk, and stand for extended periods.

  • Must be able to lift up to 25 pounds.

  • Willingness to work outdoors in varying weather conditions.
