We are seeking a Family Support Specialist 2 to provide in-home and community-based support to youth and families, fostering resilience and positive change.
Assess family needs and implement targeted, evidence-based interventions to promote lasting improvements.
Develop personalized care plans and deliver hands-on support in various settings, collaborating with treatment teams, educators, and community stakeholders.
Complete accurate documentation in electronic records, contribute to quality improvement initiatives, and uphold safety and organizational standards.
Build rapport with youth and families, demonstrating professionalism, cultural humility, and ethical decision-making.
Must be at least 21, with a relevant bachelor's degree or equivalent experience, and able to obtain background checks, First Aid/CPR, and Violence Prevention certifications. Valid driver’s license required.
Salary ranges from $20.38 to $22.94 per hour, depending on education and experience.
