Position Summary

We are seeking a dedicated Family Program Clinical Case Manager to support residents and families in our transitional housing program. This role provides a combination of case management, counseling, and recovery support, helping families achieve stability through individualized planning, community connections, and ongoing therapeutic engagement.

You'll play a key role in guiding families through their recovery journey-supporting mental health, housing transitions, and long-term independence.


Key Responsibilities

  • Facilitate new resident orientation, including move-in support and development of Individual Recovery Plans (IRPs)

  • Provide weekly counseling and case management to families, tracking progress toward goals

  • Serve as a therapist, advocate, and care coordinator, connecting residents to:

    • Mental health services

    • Substance use treatment

    • Healthcare and community resources

  • Support residents in securing permanent housing upon program completion

  • Monitor and coordinate drug screening for assigned residents

  • Maintain accurate, confidential documentation in compliance with agency standards

  • Collaborate with internal teams (program staff, employment, education, and intake)

  • Participate in case reviews, staff meetings, and supervision

  • Contribute to a team-based environment, including occasional on-call support


Qualifications

  • Master's degree in Social Work, Counseling, Psychology, Family Therapy, or related field

  • 3-5 years of counseling and/or case management experience

  • Active Maryland licensure (LMSW, LGPC, or equivalent)

  • Experience with:

    • Trauma-informed care

    • Substance use and co-occurring disorders

    • Evidence-based approaches (e.g., Motivational Interviewing, CBT)

  • Strong organizational, documentation, and communication skills

  • Ability to build therapeutic relationships and work collaboratively

  • Valid driver's license and reliable transportation

Bonus:
