Family Program Case manager & Support Specialist The goal is to provide prevention and intervention services for parents with cognitive or mental health issues and families in crisis. Direct service support will be delivered through a team-based approach for individuals with diverse mental health, developmental, and behavioral needs. Responsibilities include coordinating care, leading therapeutic activities, setting treatment goals, and ensuring comprehensive support across settings. This role fosters a positive environment that encourages growth and empowerment. Additionally, the program provides goal-oriented services that pair at-risk youth with positive role models. Its objective is to deliver support, guidance, and constructive opportunities for young individuals facing challenges in their home, school, or personal lives.
