We are hiring a Family Model Provider (Independent Contractor) in Tennessee to offer home-based support for individuals with Intellectual and Developmental Disabilities (I/DD) and mental health or substance use challenges.
As a caregiver, you'll provide personalized assistance with daily activities, nutrition, self-care, social and recreational opportunities, and coordinate medical and community services, respecting individual preferences and cultural backgrounds.
The role includes ensuring a safe, supportive home environment and participating in person-centered planning and team meetings. Support, training, and supervision are provided, along with assistance like crisis support and respite coordination.
Requirements: HS diploma/GED, 18+, reliable transportation, valid driver’s license, background checks, and proof of home ownership/renters insurance.
