Full job description
Avalon Medical Center is seeking a full time/part time/contract board certified/eligible family medicine physician to join our growing multi-specialty practice. 
 
Essential Duties and Responsibilities
  • Providing general primary care services to patients.
  • Diagnosing, treating and managing both acute and chronic conditions.
  • Prescribing and refilling medications as appropriate.
  • Actively communicating with patients, families, other specialists and facility personnel (specialists, social workers, etc).
  • Ordering and interpreting diagnostic tests such as imaging and lab work.
  • Documenting patients’ histories and treatments in EMR.
Minimum Education:
  • Degree of M.D. or D.O. - Family medicine or internal medicine
  • Board Certified or Board Eligible 
  • Current/active New York State Medical Practitioner License
Minimum Experience:
  • Professional clinical experience in primary care practice in either family medicine or internal medicine required.
Required Certification/Registration:
  • Valid NYS Medical License
  • Board Certification in Internal Medicine or Family Practice
  • Current and ongoing BLS certification required and ACLS preferred
  • NY State DEA Certification, valid and unrestricted required.
  • Current NY State Opioid Prescriber Certificate required.
  • Current Infectious disease certificate required.
Qualifications
  • Experience in patient care within a medical office setting is preferred.
  • Strong communication skills with the ability to explain complex medical information clearly to patients.
  • Ability to work collaboratively within a multidisciplinary team environment.
  • Commitment to providing high-quality patient care with empathy and professionalism.
