Job Details

We are seeking a dedicated and passionate Family Medicine Physician to join our robust healthcare team. This is a permanent, full-time role that offers an exciting opportunity to provide comprehensive care to patients of all ages. You will be working closely with a team of committed healthcare professionals to deliver exceptional patient care. The ideal candidate will be skilled in Evidence-Based Medicine, Chronic Disease Management, Preventive Care, Care Coordination, Medication Management / Prescribing, and Population Health.

Responsibilities:

  • Provide comprehensive and continuous care to patients and their families.
  • Utilize evidence-based medicine to diagnose and treat a variety of acute and chronic conditions.
  • Develop and implement patient management plans, including preventive care.
  • Coordinate patient care with other healthcare professionals and refer patients to specialists when necessary.
  • Prescribe and manage medications in accordance with patient needs and regulatory guidelines.
  • Monitor the health status of patients with chronic conditions and adjust treatment plans as necessary.
  • Educate patients and their families on disease prevention and health maintenance.
  • Participate in population health initiatives to improve the health of the communities we serve.
  • Adhere to all relevant medical laws, regulations, and standards.

Qualifications:

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.
  • Board certified or board eligible in Family Medicine.
  • A minimum of 2 years of experience in Evidence-Based Medicine, Chronic Disease Management, Preventive Care, Care Coordination, Medication Management / Prescribing, and Population Health.
  • Valid state medical license and DEA license.
  • Exceptional clinical, diagnostic, and communication skills.
  • Strong commitment to patient-centered care and a passion for improving the health of our communities.
  • Ability to work collaboratively with other healthcare professionals.
