Family Physician

Essential Job Functions

  • Provide direct patient care using the patient-centered medical home model and chronic disease management.
  • Diagnose, treat, manage, and prevent disease according to accepted standards of care, deliver high-quality care and ensure patient satisfaction.
  • Provide primary care including neonatal through geriatric ages. Provide prenatal and obstetrical care as agreed upon with clinical leadership.
  • Address urgent care needs as part of general practice, including minor procedures and emergency stabilization as needed.
  • Participate in clinical leadership activities, quality assurance/improvement, and clinical team meetings.
  • Adhere to clinical protocols and maintain comprehensive, accurate, and timely medical records in the practice EHR.
  • Participate in practice productivity and quality goals.
  • Maintain licensure, certification, and required continuing education.

Qualifications Required

  • Current, unrestricted MD or DO license in the state of practice.
  • Board certified or board eligible in Family Medicine.
  • Current DEA registration.
  • Valid BLS certification.
  • Excellent communication skills and commitment to working with underserved populations.

Preferred

  • Experience in community health centers or safety-net environments.
  • Experience with Electronic Health Records (EHR) systems.

Work Environment & Schedule

  • Full-time position (40 hours per week) with occasional Saturday coverage and after-hours phone triage on rotating schedule. On-call responsibilities may be required.
  • Work primarily in outpatient clinic settings with potential for some community outreach and home visits.
