Reason for Coverage: Provider on leave
Dates of Coverage: ASAP - January 25th
Average Number of Patients Per Day: 20-30
Number of Physicians in Practice: 2
Will the Locum be Responsible for Supervising Other Health Care Providers? No
Services Available On-Site: Laboratory, Radiology
Documentation System/EMR: AthenaHealth
Practice Population: Adult, Geriatric
Required General Procedures: EKG Interpretation
Required Surgical Procedures: Incision & Drainage (I&D), Suturing of Minor Lacerations
Required Occupational Medicine Procedures: Employment Physicals, Diagnosis & Management of Common Industrial-Related Medical Problems, Disability Exams

Malpractice Insurance: Coverage provided
Requirements:
Board Certified in Internal Medicine/Family Medicine - Required
Local providers only (mileage will be reimbursed) - Required
NJ license in hand - Required
Internal credentialing must be completed with the submitting agency - Required
Clean background - Highly preferred
BLS certification - Required
AthenaHealth experience - Highly preferred
Recent outpatient experience in NJ - Highly preferred
Credentialed with BCBS - Required
