Position Summary

The Family Medicine Physician delivers comprehensive, patient-centered primary care to individuals of all ages in a Federally Qualified Health Center (FQHC) setting. Working as part of an interdisciplinary care team, the physician plays a critical role in improving access to high-quality, affordable care with a focus on prevention, chronic disease management, and coordination of care that addresses medical, behavioral, and social determinants of health.

This role requires adherence to current clinical standards, FQHC regulatory guidelines, and organizational protocols. The physician is also expected to actively engage in quality improvement initiatives and collaborate with staff to support a culture of excellence, equity, and continuous learning.

Essential Duties and Responsibilities

  1. Provide evidence-based, comprehensive primary and preventive care to a diverse patient population.
  2. Order, interpret, and evaluate diagnostic tests to inform clinical decision-making.
  3. Collaborate with internal staff and external consultants to ensure coordinated care.
  4. Maintain complete, accurate documentation in the Electronic Medical Record (EMR) system.
  5. Participate in quality assurance and policy development initiatives.
  6. Engage in staff training and professional development as required.
  7. Represent Connect Health + Wellness with professionalism and exemplary customer service.
  8. Participate in the after-hours on-call provider rotation.
  9. Uphold patient confidentiality and privacy per HIPAA standards.
  10. Supervise and support mid-level practitioners, as needed.
  11. Maintain visitation rates in alignment within established goals.
  12. Maintain patient visit duration/cycle time within established goals.
  13. Actively participate in peer review and continuous improvement efforts.
  14. Respond to phone calls and EMR communications within 48 hours.
  15. Meet or exceed benchmarks in Clinical Performance Measures (e.g., diabetes, hypertension, cancer screening).
  16. Contribute to patient outreach and growth efforts.
  17. Perform additional duties as assigned that support the mission of the organization.
