For more information visit www.acclaimhealth.orghttpwww.acclaimhealth.org.Job Summary· Outpatient Clinic· Patient panel - Primary Care - GeriatricsEssential Duties and Responsibilities· Coordinates the multidisciplinary care deliveredto their panel of patients to optimize patient care.· Reviews patient medical records to diagnose treat and provide care management and education to patients and families.· Makes appropriate patient referrals.· Champion a culture of empathy dignity and respect for aging populations.· Monitors national health guidelines and those established by JPS Health Network and Acclaim Multispecialty Group for the management of patient care.· Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores. Demonstrates patient centered and patient valued behavior.· Maintains a collaborative environment with other referring physicians residents medical students advanced practice professionals staff and support staff to provide quality patient care.· Supports academic educational and research endeavors.· Assess and manage chronic and acute conditions in patients with multiple comorbidities.· Partners and communicates with acumen to build a positive brand and patientcustomer experience for those who encounter JPS Health Network and Acclaim Multispecialty Group. This would include health network staff vendors outside partners patients stakeholders and local community leaders.· Serves as a resource to the community.· Performs other job-related duties as assigned.
