We are seeking an experienced Registered Nurse Case Manager who is passionate about pediatric/adult care, values autonomy, and excels at building relationships with families and clinical teams.
The RN Case Manager will oversee and coordinate in-home nursing services for pediatric/adult members throughout the Phoenix metropolitan area. This hybrid position allows you to work remotely while conducting required home visits and collaborating regularly with our Phoenix office and clinical leadership team.
The ideal candidate has experience in pediatric/adult home health, private duty nursing, or pediatric/adult case management and is committed to delivering exceptional care and support to children and their families.
Conduct in-home assessments and routine member visits throughout the Phoenix metro area
Develop, implement, and update individualized plans of care
Coordinate services with physicians, DDD Support Coordinators, families, and community providers
Provide clinical oversight, education, mentorship, and support to field nurses
Monitor member outcomes and ensure quality and compliance standards are maintained
Participate in an on-call rotation to address urgent clinical needs
Attend meetings, trainings, and collaborative sessions at the Phoenix office as needed
Complete accurate and timely documentation in accordance with DNI, DDD, and regulatory requirements
Active Arizona Registered Nurse (RN) license in good standing
** Home Health experience is required (either pediatrics or geriatrics)**
BSN preferred
Strong leadership, communication, and organizational skills
Ability to work independently while managing multiple priorities
Must reside within the Phoenix metropolitan area
Reliable transportation, valid driver's license, and current auto insurance required
Comfortable traveling throughout the Phoenix service area for member visits
