Job Summary and Qualifications The Exercise Physiologist is responsible for providing individualized, goal directed care and education to patients using the systematic process of assessment, implementation, and evaluation. The Exercise Physiologist will analyze the patients' fitness to help them improve or maintain good health. They will help patients with heart disease and other chronic conditions, like pulmonary disease, regain their health. This position requires proficiency in cardiac and pulmonary care to ensure the delivery of high service standards while promoting excellent patient experience. Demonstrates appropriate judgment, priority setting skills, and decision making abilities. Consistently and effectively organizes and utilizes available resources. Communicates effectively with patients in a caring and empathetic manner regarding their diagnosis, plan of care and goals of treatment. Communicates effectively with referring physicians regarding the status of their patients receiving therapy. Completes discharge planning by consulting with physicians, nurses, social workers, and other health care workers. Attends departmental and hospital meetings, rounding (such as barrier rounds), consultations or conferences as required to contribute to patient care Works within a team to improve the effectiveness of service delivery and positive outcome measures.
