Job Description: This role will be responsible for the development, implementation, resolution and oversight of waiver incident reporting at member level and organizational level. The primary functions will include waiver incident reporting education, incident reporting, incident investigation, incident processing and closure. Position ensures Buckeye's compliance with our Three-Way and Provider Contract along with the Ohio Administrative Code and CMS regulations delegated to Buckeye through the MyCare Ohio Waiver administered through the MMP in Ohio and other required Incident Reporting in Medical Management as outlined our MCO/MCOP Provider Agreements. The MyCare Ohio Waiver is a 1915 c waiver approved by CMS for Ohio.

Purpose: The purpose of the team is to identify critical incidents for client's Health Plans Medicaid and MyCare members and ensure timely submission into the Ohio Department of Medicaid Incident Management System (IMS). This team reviews the incidents and works with the care coordination teams to create person-centered prevention plans to help ensure the health, safety and welfare of all our members.

Day to day responsibilities:
The Incident coordinator is responsible for monitoring the incident mailbox and triaging any incidents that come in and forwarding any emails related to open cases. They will be responsible for reviewing the Critical Incident Report daily and checking for previous incidents. Assigning the cases appropriately and placing the information on the internal tracking sheet. The coordinator will be responsible for following all MyCare Waiver and MyCare SRS cases and uploading all documents into the IMS system.
Staff performance is measured by timeliness of submission, timeliness of submitting Prevention Plans and closing Reportable cases; as well as accuracy of incident submission and PPs.
This role is unique to Ohio. Only Ohio tracks incidents in conjunction with the state
This is a fully remote position; however, the team is very cohesive and in frequent contact with me and each other. The team freely offers assistance to each other as well as other internal and external customers. We have a strong focus on accountability, honesty and service.

Candidate Requirements
Education/Certification Required: Degree and or certificate/licensure required in a field of Nursing, Social Work and/or Counseling Preferred:
Licensure Required: Must have a OHIO licensure as an LPN, SWA, CHW, LSW or LPC. Preferred:
Years of experience required: 2 years experience managed care; must have knowledge of Ohios MyCare Waiver & OH incident reporting . Nursing, Social Work and/or Counseling.

Disqualifiers: No knowledge of MyCare Waiver & incident reporting
