Upon an offer being extended and accepted to your candidate- please send copies of the following to the Program Specialist assigned to your requisition for credentialling: -Copy of the candidates resume. -Copy of Official Transcripts IN ORDER FOR CANDIDATES TO QUALIFY THEY MUST MEET THE CLIA STANDARDS FOR TESTING PERSONNEL: [Candidates must possess at least 1 of the below] -Bachelors Degree in Medical Technology or Clinical Laboratory Science OR -BS in Chemical, Physical, or Biological Science from an accredited college or university WITH at least 1 year of training or experience in specialty in which testing will be performed. Such training must be equivalent to that received in a school of Medical Technology. OR -90 Semester hours from an accredited institution that include the following: 16 semester hours of Biology courses, 16 Semester ours of chemistry [must include atleast 6 hours of inorganic Chemistry], and 3 Semester hours of Math- along with having completed an accredited Clinical Laboratory training program or 1 year of documented Clinical Laboratory Training, or experience in the specialty that testing is to be performed. OR -Associates Degree in Medical Technology. The Program Specialist will ensure to submit the credentialling form - and send all needed credentialing documents for review. | ||||||||||
