Educational Coordinator – Hospice & Home Health
POSITION SUMMARY
The Educational Coordinator provides clinical education, onboarding support, and ongoing competency development for hospice and home health staff. This role supports regulatory compliance, quality patient care, and professional growth by delivering education aligned with CMS requirements, agency policies, and best clinical practices. The Educational Nurse is skilled in OASIS or has knowledge in OASIS, documentation standards and supports staff education to improve accuracy, compliance, and outcomes.
KEY RESPONSIBILITIES:
Clinical Education & Training:
- Develop and deliver clinical education for hospice and home health staff.
- Support new-hire orientation and onboarding.
- Provide hospice and palliative care education.
- Educate staff on home health standards, skilled nursing services, and care coordination.
- Provide education and guidance related to OASIS documentation, scoring, and best practices.
Competency & Skills Validation:
- Conduct initial and ongoing competency assessments.
- Validate clinical skills per policy.
- Identify learning gaps and recommend education plans.
- Support competency related to OASIS assessments and clinical documentation.
Regulatory & Compliance Support:
- Educate staff on CMS Conditions of Participation.
- Support OASIS and hospice eligibility education.
- Assist with survey and audit preparation.
Quality Improvement & Clinical Support:
- Support quality improvement initiatives.
- Provide documentation education and feedback to improve OASIS accuracy and compliance.
- Serve as a clinical resource.
Administrative & Program Support:
- Maintain training and competency records.
- Assist with educational materials and policies.
- Participate in meetings and in-services.
REQUIRED QUALIFICATIONS:
- Current RN license.
- 2–3 years hospice and/or home health experience.
- Demonstrated skill in OASIS documentation and education.
- Strong teaching and communication skills.
- EMR proficiency.
PREFERRED QUALIFICATIONS:
- BSN preferred.
- Prior education or preceptor experience.
- OASIS certification
- Hospice or home health certification (CHPN, HCS-D, or similar).
WORKING CONDITIONS:
- Office, virtual, and occasional field-based work.
PHYSICAL REQUIREMENTS:
- Ability to sit, stand, walk.
- Ability to lift up to 25 lbs.
