Description
  • Comprehensively assess patients in a variety of settings - both inpatient and outpatient - and create personalized treatment strategies
  • Deliver physical therapy interventions tailored to enhance mobility, strength, balance, and overall functionality
  • Collaborate closely with a diverse team, including nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
  • Guide patients and their families through therapeutic goals, discharge planning, and at-home exercise regimens
  • Accurately document patient evaluations, treatments provided, and progress in alignment with facility policies and regulatory standards
  • Engage in care team rounds and interdisciplinary meetings to ensure a cohesive approach to patient care
Education
  • Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution
Certifications
  • Active state licensure as a Physical Therapist
  • CPR certification is preferred
  • Specialized certifications (e.g., neurological, geriatric) are beneficial, though not mandatory
Skills
  • Prior experience is highly valued
