Job Summary and Qualifications

Full time (Monday - Friday, day shift), no nights, no on-call, no weekends, no holidays 

 DUTIES INCLUDE BUT NOT LIMITED TO:

• Assesses and evaluates medical record documentation to ensure all physician documentation is completed in compliance with established guidelines and uploaded to the correct patient’s medical record in a timely and accurate manner.

• Reviews work queue at the beginning of the day to identify batches to process for assigned practices, and continues to monitor throughout the day for incoming batches.

• Assists with batches for other practices during heavy workloads and team member absence.

• Notifies management when practices are not scanning.

• Re-names incoming batches with the correct name of the corresponding practice.

• Indexes all imaged pages by appropriately selecting correct patient MRN and encounter.

• Creates scanned document encounters as needed.

• Assigns appropriate document type to all images ensuring documentation presents under the correct chart review tab. o Verifies images are correct and legible.

o Ensures that each scanned document is associated to the appropriate patient.

o Ensures that each scanned document is associated to the appropriate patient account.

o Ensures that each scanned document is associated to the appropriate document type.

o Performs a paper document to PC screen validation to ensure that all documents associated with each record have been scanned.

 

• Associates all clinical results with corresponding order, routing results to ordering provider’s in- basket.

• Creates orders as appropriate to meet Best Practice Advisories (BPAs).

• Writes (saves) each batch to the appropriate queue and uploads to Epic.

• Identifies documents not approved for retention to the electronic health record and documents to be returned to the practices for follow-up and forwards to the Quality Manager and/or HIM team leader.

• Monitors the Fatal Error work queue and the Index Correction Utility (ICU) to identify documents that fail to import/upload into the designated system (MPF/EPIC). Investigates and resolves issues allowing documents to successfully import into designated system.

• Monitors Batch Error queue and Mag Error queue to identify any batches failing and resolves issue. Investigates and resolves issues allowing documents to successfully import into designated system.

• Identifies trends associated with scanning issues for further education to be provided to the practices.

• Identifies incorrect medical record numbers assigned and other issues requiring follow-up and education with HIM Analyst. Reports to Quality Manager and/or HIM team leader.

• Processes paper medical records for storage at offsite facility. Maintains master log for all records processed.

• Requests records from off-site storage, updates spreadsheet for the location of record, contacts courier for delivery of record and coordinates return of record to off-site storage facility.

• Serves as a preceptor for new team members and co-workers.

• Follows through and assists in resolving issues.

• Prepares daily statistical reports reflecting work volumes and records daily work volumes on master spreadsheet.

• Utilizes daily communication with peers to ensure continuity of services.

• Presents oneself in a professional, positive manner, possessing thorough knowledge of Health Information Management functions.

• Performs related work as required.

• Practice and adhere to the Code of Conduct and Mission and Value Statements.

 

EDUCATION:

• High school diploma required or equivalency; Associate degree preferred.

 

EXPERIENCE:

• Minimum of 1 year experience in a healthcare setting. Experience in HIM preferred.

 

CERTIFICATION/LICENSE:

• RHIT, RHIA, or CRIS certification preferred.
