Job Description:
Demonstrates ability to request care from referring sources within the community in a timely manner. Reviews patient medical records for Performance Improvement/Risk Management per established criteria and document adverse patient occurrences. Collects information and documents patient and family reasons for readmission. Documents discharge plans, MDRs, barriers to progression of care, avoidable days, and patient and family needs according to standard work. Provides patient and family advocacy, and support patient's choice and patient rights during hospitalization. Communicates with Payors patient's needs for authorization for post-acute care as needed. Other duties as assigned. Collects information from patients and families to inform discharge planning needs in the inpatient, outpatient, and/or emergency departments. Collaborates with the RN Care Manager to reviews the medical record and incorporates the clinical, social, and financial factors into the discharge planning. Arranges for post-acute services and facilities as well as communicate care for social needs. Leverages technology and follows standard work and best practices to communicate with post-acute care services and facilities to ensure patient care information is communicated for continuity of care, medical records are complete, and discharge reconciliation is accurate. Communicates with patients and families and mobilizes family and community resources to meet patient's needs while advocating for patient and family empowerment in making health care decisions and accessing needed services.
