Job Summary

Carroll Hospital is seeking a Discharge Support Specialist to support the Care Management Department through essential administrative and operational functions. This role plays a key part in meeting regulatory requirements, supporting patient throughput, and ensuring patients receive appropriate post-discharge services.

The Discharge Support Specialist serves as the administrative liaison between clinical departments, community resources, and third-party payers/insurance companies. The position ensures patients are informed of their rights and available choices throughout the discharge process, contributing to safe, timely, and effective transitions of care.

Key Responsibilities

  • Provide administrative and operational support to the Care Management Department

  • Assist with patient throughput activities and discharge coordination

  • Support regulatory and documentation requirements

  • Communicate with clinical teams, community resources, and insurance providers

  • Ensure patients receive information regarding post-discharge services, rights, and options

  • Maintain accurate records using electronic medical record (EMR) systems

  • Utilize office equipment and Microsoft Office applications effectively

Qualifications & Requirements

Education

  • High School Diploma (preferred)

  • Associate's Degree (preferred)

Experience

  • Required:

  • 1-3 years of clerical experience in a healthcare setting

  • Experience using Microsoft Office Suite and electronic medical record systems

  • Preferred:

  • Less than 1 year of experience in care coordination, care management, disease management, hospital/acute care settings, or customer service

Specific Requirements

  • Familiarity with medical terminology
