General Description: Under the general supervision of Mission Control, the Discharge RN is responsible for facilitating a patients safe transition to home or another setting at hospital discharge. The RN, utilizing the Person-and-Family-Centered Care Model, recognizes and addresses family needs and preferences, and integrates family caregivers as partners in care demonstrating mutual trust and respect.
The role includes reviewing discharge orders, providing patient and family education on medications, follow-up appointments, and self-care instructions, and ensuring all required documentation is complete. The Discharge RN collaborates with physicians, case managers, social workers, and other care partners to reduce re-admissions, promote continuity of care, and supports patient admissions to the unit to support patient and hospital needs. The position shall exemplify the desired Culture of Choice and philosophies of Parrish Healthcare.
Key Responsibilities:
• Review provider discharge orders for accuracy and completeness and clarify discrepancies as needed. • Provide comprehensive patient and family education on medications, treatments, follow-up appointments, warning signs, and self-care instructions. • Instruct patients and families on topics such as health education, disease prevention, and answer patient questions to make informed decisions about their health and treatment. • Use tools such as teach-back to confirm understanding. • For admission of patients, complete a physical and psychosocial assessment including growth and development of the patient. • Record and analyze patient medical history, medication history, symptoms, and conditions. • Monitor, record, and report symptoms or changes in patients conditions. • Evaluate the effectiveness of planned interventions in relation to patient responses and the attainment of expected outcomes. • Document and use appropriate communication actions to maintain continuity. • Communicate effectively with care partners, physicians, patients, families, and other healthcare team members. • Includes the person/family to actively participate in face-to-face bedside handoff/handover report; communicates in SBAR format for all handoff/handover reports and changes in patients status to the Care Team. • Follows the chain of command. • Documentation is concise, pertinent, and legible, in the appropriate form and according to accepted standards. • Nursing care is provided in an organized, timely, safe, cost-effective manner. • Participates in Quality Monitoring/Performance Improvement. • Incorporates Evidence-Based practices into daily care. • Performs similar or related duties as assigned. • Knows fire, disaster, and safety procedures and regulations as it pertains to the work area.
Requirements:
• Formal Education: Graduate of an RN education program (accredited preferred); Associate Degree in Nursing required. Bachelor Degree in Nursing preferred. • Work Experience: At least 2 years experience. • Licenses, Certifications, Registrations: State of Florida RN *BLS, ACLS, PALS certifications must be obtained through the AHA.
