BayCare Health Systems is currently seeking a Discharge Planner 1 to join our outstanding and compassionate team at St. Joseph's North!

- Shift Hours: 8:00 am- 4:30 pm

The Discharge Planner I responsibilities include:
- Provides assessment, linkage, monitoring, planning and advocacy for adult and or children experiencing acute mental health and or substance abuse issues in an acute care inpatient setting.
- Possesses a clear understanding of discharge and transition planning and linkage with community resources.
- Oversees legal status issues related to the individuals involuntary or in accordance with the Baker Act regulations.
- Provides support as active member of the treatment team.
- Will perform other duties as assigned by supervisor.
Travel may be required.
Required Certification:
Required Experience:
- Behavioral Health - 1 year
- Child and Adolescent - 2 years (SIPP Only)
- Acute Care - Preferred 1 year
Required Education:
- Bachelors - Counseling/Health Education/Related Field/Social Work/Human Services/Psychology/Nursing
- Master's in human services - Preferred
