Position Summary
The RN Discharge CoordinatorCase Manager performs a variety of duties centered around the discharge and case management of patients. Responsibilities include assessing patients for transitional care needs and barriers to discharge communicating with the patient and family on their preferences charting and documentation of plan of care coordinating plans for discharges involving community resources and alternate levels of care facilitating successful adherence to the discharge plan maintaining high quality patient centered and cost-effective results and complying with all regulatory requirements. The Discharge CoordinatorCase Manager has a pivotal role in providing a smooth transition to the patients next level of care. This role requires the ability to utilize critical thinking multitasking and prioritization. An additional requirement is strong communication skills to promote collaborative relationships with the patients their families and the healthcare team.
Qualifications
Education Bachelors degree in nursing or allied health field preferred
Experience 2 years of progressive nursing experience in an acute care setting preferably within discharge utilization andor case management - or the equivalent in education andor experience - is required Please note Preference will be given to candidates having both preferred experience and education
LicensureCertifications RN Registered Nurse licensed by the Oregon State Board of Nursing required upon startCCM Certified Case Manager preferredACM Accredited Case Manager preferred
