Overview

At Larimer County Department of Human Services, we strive to be responsive, provide timely resources in partnership with our community so individuals and families are healthy, supported, and safe. We are seeking a highly motivated, innovative, and enthusiastic staff member to join our Operations and Organizational Development Division as a Case Manager I/II.

The Disability Navigator team is responsible for assisting Larimer County residents (both adults aged 18-59 and children in Larimer Countys Children, Youth, & Family custody) in navigating the Social Security Administration (SSA) disability screening and benefit process.

The typical schedule for this position is Monday-Friday, 8 AM - 5 PM. This position is a hybrid position and will require at least one day/week in the office. You will be required to have adequate internet access and space for remote work. Larimer County provides a one-time remote work stipend to assist with equipment and office supply needs for remote workspaces. Three professional references with valid contact information are required to be submitted as part of this application.

Responsibilities

What youll be doing:

  • Assisting in developing a complete, thorough, and quality application for Social Security Disability Insurance (SSDI) and/or Supplemental Security Income (SSI) benefits when requested by the client and/or serving as the client's Appointed Representative with the SSA.
  • Assisting in supporting clients through SSA appeals, denials, or hearings when requested by the client and/or serving as the client's Appointed Representative with the the Social Security Administration (SSA).
  • Completion of SOAR (SSI/SSDI Outreach, Access, and Recovery) certification upon hire (online training course for case managers assisting individuals in applying for the SSAs disability programs).
  • Outreaching to clients to provide reminders and track progress on SSA applications and appeals.
  • Assisting in obtaining required medical documentation through referrals to appropriate medical providers and other professionals whose assessments are required as part of an application for federal disability benefits.
  • Participating in relevant State and Regional committees and trainings related to disability navigator program.
  • Collecting and tracking program data, submitting required reports in a timely manner.

Qualifications

All candidates will be screened for the following required minimum qualifications. Any candidate who does not meet all of the qualifications listed as required will be eliminated from the process.

Depending on qualifications this position may be hired as a Case Manager I or II.

Case Manager I $24.1868/hr - $29.0241/hr

  • Equivalent to a Bachelors Degree from an accredited college or university required. Combination of education and/or experience may be substituted on a year for year basis.
  • Possession of a valid Drivers License by date of hire required.
  • Complete training and/or certification as mandated by the assigned program required.

Case Manager II $26.6055/hr - $31.9266hr

  • Equivalent to a Bachelors Degree from an accredited college or university required. Combination of education and/or experience may be substituted on a year for year basis.
  • One (1) year of experience in a directly related field or in the performance of similar duties and responsibilities required.
  • Possession of a valid Drivers License by date of hire required.
  • Complete training and/or certification as mandated by the assigned program required.

Supplemental Information

This position is limited term. Individuals hired into limited term positions are eligible for County benefits, however, the position is limited in duration dependent upon available funding or business need. The position will be located at one of the following three locations: 1501 Blue Spruce Drive, Fort Collins, 2555 Midpoint Drive, Fort Collins, or 200 Peridot Avenue, Loveland.

All new employees and rehires will be required to submit fingerprints for a CBI criminal background check (c.r.s. 26-6-104).

All staff working in the Options for Long Term Care Unit, Adult Protection Unit, or Office on Aging will be required to submit information for a Colorado Adult Protective Services data system check (c.r.s. 26-3-1-111).

An offer of placement is contingent upon the successful completion of required pre-employment checks or testing.
