Mission Creek, part of Vista Senior Living Management, is seeking a strong Director of Wellness / Clinical Leader to lead clinical services for our established CBRF and Memory Care assisted living community in Waukesha, WI.
This is a great opportunity for a clinical leader who enjoys assisted living, relationship-based care, resident and family communication, staff development, and building a strong care culture.
We strongly prefer an already licensed Registered Nurse (RN). However, we are also open to experienced LPN candidates with strong CBRF/assisted living leadership experience who are actively pursuing their RN.

Unlike a traditional skilled nursing DON role, this position is focused on assisted living and memory care operations, including resident wellness, service plans, clinical oversight, family communication, regulatory compliance, and team leadership.
What You’ll Do
As Director of Wellness, you will oversee the day-to-day clinical operations of the community with support from an Assistant Director of Wellness, Resident Care Coordinator, and caregiving team.
Responsibilities include:
- Lead, coach, and support clinical and direct care staff
- Oversee resident assessments, service plans, and clinical documentation
- Monitor changes in resident condition and coordinate appropriate follow-up
- Communicate with families, physicians, hospice, home health, pharmacies, and outside providers
- Support admissions, pre-admission screenings, and resident transitions
- Partner with the Executive Director and leadership team on quality, compliance, staffing, and resident satisfaction
- Participate in service plan meetings, incident review, quality improvement, and staff education
- Help build a positive, accountable, and team-oriented care culture
What We’re Looking For
- Active Wisconsin RN license strongly preferred
- Experienced LPN candidates with strong CBRF/assisted living leadership experience and active pursuit of RN licensure may also be considered
- Assisted living, memory care, CBRF, long-term care, or geriatric nursing experience preferred
- Prior nurse leadership experience preferred
- Strong communication, organization, and follow-through
- Ability to coach, support, and develop caregivers and clinical leaders
- Comfort working directly with residents, families, physicians, and outside providers
- Strong clinical judgment and commitment to quality care
- CPR certification
