Heather Hill Health Care Center is seeking an experienced and compassionate Director of Social Services to join our leadership team.

This is a key leadership role within our skilled nursing facility focused on supporting residents and families through every stage of their healthcare journey—from admission through discharge planning, while promoting dignity, emotional well-being, and quality of life.

We are looking for a strong social services professional with Skilled Nursing Facility (SNF) or Long-Term Care experience REQUIRED who understands the importance of resident advocacy, regulatory compliance, care coordination, and interdisciplinary collaboration.

What You’ll Do

  • Resident & Family Advocacy
  • Support residents and families throughout the admission, stay, and discharge processes
  • Conduct psychosocial assessments and identify emotional, social, and support needs
  • Advocate for resident rights, dignity, and quality of life
  • Assist families with transitions of care and community resources
  • Care Planning & Interdisciplinary Collaboration
  • Participate in interdisciplinary care plan meetings and resident care conferences
  • Collaborate closely with nursing, therapy, admissions, and leadership teams
  • Help coordinate safe and appropriate discharge planning and care transitions
  • Address resident concerns, behavioral needs, and family communication
  • Leadership & Compliance
  • Oversee social service programming and department operations within the facility
  • Maintain accurate documentation in compliance with federal, state, and facility regulations
  • Provide leadership, guidance, and support to social services staff
  • Support survey readiness and regulatory compliance initiatives

What We’re Looking For

  • Bachelor’s Degree in Social Work or related field REQUIRED
  • Current Florida licensure (LBSW, LCSW, or equivalent) REQUIRED
  • Minimum 2 years Skilled Nursing Facility (SNF) or Long-Term Care experience REQUIRED
  • Strong understanding of federal and state SNF regulations
  • Excellent communication, documentation, and organizational skills
  • Compassionate, resident-centered approach to care
  • Ability to navigate difficult family conversations and care transitions professionally
