We are seeking an experienced leader to oversee social services, admissions coordination, and discharge planning within a skilled nursing setting. This role supports residents and families while ensuring smooth transitions and regulatory compliance.
Compensation: $25/hour
Direct social services operations in a skilled nursing facility
Oversee admissions processes and discharge planning
Support residents and families with care planning and resource coordination
Ensure compliance with Medicaid and applicable regulations
Collaborate with the interdisciplinary team to promote positive outcomes
Bachelor's degree in Social Work or related field required
LSW preferred
Knowledge of Medicaid and long-term care processes required
Skilled nursing or long-term care experience preferred
Prior leadership or supervisory experience preferred
