Job Summary:We are seeking an experienced Director of Rehabilitation to lead a dynamic therapy team within a skilled nursing facility setting. This leadership role is ideal for a licensed therapist looking to advance their career while continuing to positively impact patient outcomes in both short-term rehabilitation and long-term care.

Key Responsibilities:

• Oversee daily operations of the rehabilitation department
• Develop, implement, and monitor individualized patient treatment plans
• Supervise and mentor therapy staff (PT, OT, ST, PTA, COTA)
• Ensure compliance with clinical standards and regulatory requirements
• Collaborate with interdisciplinary teams to improve patient outcomes
• Maintain high standards of care, documentation, and service delivery

Requirements:

• Active or eligible licensure in South Dakota as a PT, OT, ST, PTA, or COTA
• Graduate of an accredited therapy program
• Minimum 2 years of experience (SNF or rehab setting preferred)
• Strong leadership, communication, and team collaboration skills

Work Environment:

• Skilled Nursing Facility (SNF)
