Position Summary

Brownfield Health & Rehab Center is seeking an experienced and compassionate Director of Rehabilitation to lead our therapy department and oversee the delivery of high-quality rehabilitation services. The Director of Rehabilitation is responsible for the clinical, operational, and financial management of rehabilitation programs, ensuring exceptional patient outcomes while maintaining compliance with all federal, state, and facility regulations.

The ideal candidate will be a licensed PT, PTA, OT, COTA, or SLP with strong leadership experience and a commitment to delivering exceptional rehabilitation services in a skilled nursing environment.

Essential Duties and Responsibilities

  • Direct and manage all rehabilitation services, including Physical Therapy, Occupational Therapy, and Speech Therapy programs.
  • Supervise, mentor, coach, and evaluate rehabilitation staff to ensure quality patient care and professional growth.
  • Coordinate interdisciplinary treatment plans to maximize patient outcomes and functional independence.
  • Monitor departmental productivity, staffing, scheduling, and operational efficiency.
  • Ensure compliance with Medicare, Medicaid, PDPM, state regulations, accreditation standards, and facility policies.
  • Review and maintain accurate clinical documentation and therapy records.
  • Collaborate with physicians, nursing staff, case managers, and other healthcare professionals to coordinate resident care.
  • Participate in admissions evaluations and determine rehabilitation service needs.
  • Manage department budgets, equipment, and resource utilization.
  • Analyze quality measures, outcomes, and performance metrics to identify improvement opportunities.
  • Maintain a positive work environment that promotes teamwork, accountability, and resident-centered care.
  • Serve as a liaison between facility leadership, therapy staff, residents, and family members.

Qualifications Required

  • Current Texas licensure as one of the following:
  • Physical Therapist (PT)
  • Physical Therapist Assistant (PTA)
  • Occupational Therapist (OT)
  • Certified Occupational Therapy Assistant (COTA)
  • Speech-Language Pathologist (SLP)
  • Minimum of three (3) years of rehabilitation management, supervisory, or leadership experience.
  • Experience in a skilled nursing facility, post-acute care, rehabilitation hospital, or long-term care setting.
  • Strong knowledge of Medicare, Medicaid, PDPM, therapy reimbursement, and regulatory compliance requirements.
  • Excellent leadership, communication, organizational, and problem-solving skills.
  • Ability to manage multiple priorities while maintaining high standards of patient care.
  • Proficiency with electronic medical records (EMR) and therapy documentation systems.

Preferred

  • Previous experience as a Director of Rehabilitation, Assistant Director of Rehabilitation, or Lead Therapist.
  • Experience with census development, program growth, and quality improvement initiatives.
  • Proven success in improving clinical outcomes and departmental performance.
