The Staff Pad is partnering with a Federally Qualified Health Center in Ontario, CA, to recruit a results -driven Director of Quality Improvement to join its leadership team and support its mission of delivering high -quality health care to underserved communities.<\/span><\/span>
<\/p>

The Director of Quality Improvement leads the organization's Quality Improvement (QI) Program, providing strategic leadership for quality initiatives, performance measurement, and regulatory reporting to improve clinical outcomes, ensure compliance, and foster a culture of continuous quality improvement.<\/span><\/span>
<\/p>

What You’ll Do<\/span><\/span>
<\/p>

  • Lead Quality Improvement (QI) programs to improve clinical outcomes, patient safety, health equity, and operational performance<\/span>
    <\/span><\/li>
  • Drive continuous improvement initiatives using PDSA, Lean, and data -driven methodologies<\/span>
    <\/span><\/li>
  • Monitor and optimize UDS, HEDIS, Value -Based Care, P4P, and regulatory quality metrics<\/span>
    <\/span><\/li>
  • Oversee quality reporting, population health analytics, dashboards, and performance monitoring<\/span>
    <\/span><\/li>
  • Identify care gaps and implement strategies to improve patient and organizational outcomes<\/span>
    <\/span><\/li>
  • Partner with clinical and executive leadership to advance quality and population health goals<\/span>
    <\/span><\/li>
  • Ensure compliance with HRSA, FQHC, UDS, and other regulatory requirements<\/span>
    <\/span><\/li>
  • Lead audit readiness, accreditation activities, and regulatory reviews<\/span>
    <\/span><\/li>
  • Supervise and develop QI staff through performance evaluations and training<\/span>
    <\/span><\/li>
  • Partner with the CMO and COO to maintain the Quality Improvement Plan & Calendar<\/span>
    <\/span><\/li>
  • Facilitate QI Committee and clinic Quality Team meetings<\/span>
    <\/span><\/li>
  • Foster a culture of collaboration, accountability, and continuous improvement<\/span>
    <\/span><\/li>
  • Represent the organization in external quality and community partnerships<\/span>
    <\/span><\/li>
  • Ensure accurate data collection, reporting, and regulatory submissions across multiple systems<\/span>
    <\/span><\/li>
  • Support care coordination, population health initiatives, and quality improvement strategies informed by patient and staff feedback<\/span><\/span>
    <\/li>
  • Perform other duties as assigned<\/span>

    What We Are Looking For<\/span><\/span>

    Required Qualifications<\/span><\/span>
    <\/p>

    • Bachelor's degree in Healthcare Administration, Public Health, Nursing, Business, or related field; Master's degree preferred<\/span>
      <\/span><\/li>
    • 2–4 years of healthcare quality improvement experience, preferably in an FQHC or community health setting, with leadership experience<\/span>
      <\/span><\/li>
    • Knowledge of HEDIS, UDS, Value -Based Care, P4P, quality improvement, and healthcare reporting<\/span>
      <\/span><\/li>
    • Experience with data analysis, population health platforms, Power BI, SQL, and reporting tools<\/span>
      <\/span><\/li>
    • Advanced proficiency in Microsoft Office applications and data reporting tools<\/span>
      <\/span><\/li>
    • Strong leadership, communication, project management, and organizational skills<\/span>
      <\/span><\/li>
    • Ability to manage multiple priorities, work independently, and travel between locations as needed<\/span>
      <\/span><\/li>
