Responsibilities:
The Director of Patient Outcomes (Rehab) is the primary individual responsible for implementing and coordinating the patient assessment and evaluation process for all patients treated by the staff in the Rehabilitation Department. Uses his or her expertise and training in rehabilitation services and independent discretion and judgment to supervise and manage the completion of the Patient Assessment Instrument for each rehabilitation inpatient. Provides oversight and clinical support in completion of data contained in the patient assessment (PAI) and Quality Indicator data for each patient to ensure accuracy in the scoring of functional status and reflection of the patient’s care needs. Direct Case Management Department, as needed. Integrates the hospital’s mission and “Guiding Principles” into daily practice.
