Director of Nursing

Location: Hybrid (Remote & On-Site) – Client Homes in NJ | 23 Orchard Road, Skillman, NJ 08558
Start Date: April 10, 2026 


We are seeking a compassionate and detail-oriented Registered Nurse (RN) - Home Health Care to support our home care agency on a per-case basis. This flexible, field-based role involves conducting in-home client assessments, developing individualized care plans, supervising and evaluating CHHAs, and performing follow-up visits for active clients as needed.


This position offers the opportunity to play a key role in maintaining high standards of care and to transition into a full-time clinical leadership role.


Compensation:
Compensation is provided per completed service, including:


  • In-Home Client Assessments (Initial Evaluation) - $80 per assessment
  • Re-Evaluation/Follow-Up Visits - $60 per assessment 
  • Caregiver Training and Skills Evaluation - $38 to $45 an hour.
This position operates on a per-visit basis with flexible scheduling; minimum hours are not guaranteed


Key Responsibilities:
  • Clinical Evaluations: Perform initial and ongoing physical and mental health assessments, including environmental safety checks (e.g., fall risks, sanitation, and home hazards). 
  • Care Planning:  Develop, implement, and update individualized care and service plans based on client needs and clinical findings. 
  • Patient & Family Education:  Provide education on disease management, medication awareness, self-care techniques, and home safety procedures. 
  • Caregiver Supervision & Training: Provide clinical guidance, skills training, and performance feedback to caregivers to ensure safe and effective care delivery. 
  • Care Compliance Monitoring: Conduct supervisory visits to monitor care quality and ensure care plan adherence. 
  • Incident Reporting & Risk Management:
    Identify, document, and report incidents, safety concerns, or changes in client condition following agency protocols. 
  • Documentation & Compliance: Maintain accurate documentation of client visits, assessments, trainings, and evaluations in accordance with agency and regulatory requirements. 
  • Regulatory Compliance: Ensure services comply with New Jersey State home care regulations and agency policies
  • Clinical Operations Support: Support agency launch through operational development and community outreach. 
Qualifications:
  • Active Registered Nurse (RN) license in the State of New Jersey (Required)
  • 1+ years of Nursing experience, preferably in home care, community health, or case management
  • Current BLS/CPR certification (Required)
  • ACLS and/or PALS certification (Preferred)
  • Bachelor of Science in Nursing (BSN) preferred; MSN is a plus
  • Experience with clinical assessments, care plan development, and patient education.
  • Prior experience in a supervisory, case management, or caregiver oversight role preferred.
  • Strong clinical documentation skills and knowledge of healthcare regulatory compliance
  • Valid NJ Driver’s License, reliable transportation, and ability to travel locally for in-home visits; must meet required medical and background clearance guidelines
Physical Requirements:
  • Ability to sit, stand, and work on a computer for extended periods
  • Ability to occasionally lift or carry up to 15 lbs (e.g., nursing bag or equipment)
Growth:
As the agency scales, this role may expand into a full-time supervisory or clinical director position. You will have the opportunity to grow with us and help define the care standards, training protocols, and service delivery model of a mission-driven, community-based agency.
