A bit about us:

We are a large skilled nursing and rehabilitation facility in Queens entering a major new chapter.

The building is established. The staff is loyal. The residents and families are connected to the place. And now the facility is being upgraded, renovated, and repositioned for a higher-end short-term rehab future.

We are not looking for someone to hide in an office and sign off on reports.

We need a clinical leader who can walk the building, earn trust, hold standards, develop people, prepare for survey, and lead change without turning the place upside down just to prove they are in charge.

The right DON will have support around them, including ADON coverage, strong unit managers, regional resources, and an Administrator who stays involved.

This is a serious seat for a serious nursing leader.

Not a rescue mission. Not a revolving door. Not a “good luck, the survey team is here” situation.

This is a chance to help lead a 280-bed Queens facility into its next era.

Job Details

Key responsibilities
  • Lead nursing operations across a large skilled nursing and rehabilitation facility.
  • Maintain compliance with CMS, NYSDOH, and long-term care regulatory standards.
  • Drive survey readiness and quality improvement initiatives.
  • Oversee QAPI, clinical education, infection prevention, risk management, and performance improvement efforts.
  • Mentor, coach, and develop nursing leadership
  • Monitor staffing, scheduling, clinical outcomes, documentation, and resident satisfaction.
  • Partner with the Administrator and interdisciplinary team to improve systems, communication, and accountability.
  • Support short-term rehab growth by strengthening clinical quality, hospital relationships, and discharge outcomes.
  • Serve as a visible leader throughout the building.

What we are looking for
  • Active New York RN license.
  • Experience as a Director of Nursing in skilled nursing, long-term care, subacute rehab, or post-acute care.
  • Strong knowledge of CMS regulations, NYSDOH requirements, survey readiness, QAPI, infection control, staffing, and nursing home operations.
  • Proven ability to lead teams, improve outcomes, and hold people accountable without destroying morale.
  • A calm, confident leadership style.
  • Someone who can lead change in a building that is ready for its next level.
