The Director of Patient Navigation will drive operational excellence for HCA’s navigation program, specifically ensuring delivery of high quality, evidence-based care through program development under the direction of the Care Navigation leadership team and in collaboration and alignment with Corporate and Division Service Line leadership teams. The Director will serve as a regional leader for executing mid and long-range strategic initiatives to optimize operational performance and coordinate various operational tactical plans while tracking performance against key performance indicators. He/she will provide expertise, initiative, leadership and programmatic oversight to division and facility leadership across navigation programs in their respective region to elevate navigation program performance and ensure high-quality healthcare services for HCA patients. The Director will provide oversight and leadership to the Manager and Supervisors (as applicable) of Care Navigation and further support for both local and remote nurse navigators.
Patient navigation programs function to support the navigation of patients through complex healthcare environments for the optimization of care and outcomes, while maximizing patient retention. Navigators facilitate patient follow-up through coaching, information transfer, and communication in order to improve continuity of care, enhance the medical management of complex disease, and streamline the intervention process. Navigators guide patients through the health care system by identifying barriers to care, linking to needed resources, and ensuring adherence to medical treatment plan.
