Position Title: Director of Memory Care
Overview: The Director of Memory Care leads the Memory Care program and provides oversight and support to the Memory Care Manager and teams, delivering person-centered, dignity-preserving care for residents living with dementia. Champions person-centered dementia care, creating a secure and engaging environment while leading specialized programming and staff training initiatives. Responsible for maintaining a therapeutic environment that supports both residents and families while promoting excellence in dementia care delivery. This position reports to the Senior Director of Health Services.
ESSENTIAL JOB FUNCTIONS
- Design and implement memory care programming and individualized care plans — Conduct regular care audits, implement personalized interventions, and monitor clinical outcomes for continuous improvement.
- Train staff on dementia best practices, redirection, and safety protocols
- Ensure secure environment, elopement prevention, and behavioral support
- Engage families with education, support groups, and care conferences — Conduct regular care audits, implement personalized interventions, and monitor clinical outcomes for continuous improvement.
- Monitor outcomes and compliance with state regulations and company policies
- Represent the community through professional demeanor, communication and appearance
- Serve as a liaison within the greater community including hospitals, rehabilitation facilities, medical providers and external partners
- Support internal and external marketing efforts for Memory Care services and occupancy growth
- Strong communication and collaboration skills to interact with residents, families, physicians, memory care team and peers
Additional Responsibilities
- Adhere to all company policies and procedures
- Attend required education, training, and in-service programs
- Maintain confidentiality of resident, family, and organizational information
- Respond to fire alarms and assist with evacuation procedures when necessary
- Cooperate with all departments to support overall community operations
- Represent the organization through professional demeanor, communication, and appearance
- Provide exceptional customer service to residents, families, guests, and staff
- Perform additional duties as assigned

- Strong collaboration skills with residents, families, physicians, staff, and leadership

PHYSICAL REQUIREMENTS
- Ability to bend, stoop, sit, stand, walk, reach, and lift up to 50 pounds with or without accommodation due to prolonged standing and walking throughout the community while conducting rounds and responding to resident needs
- Ability to react and respond quickly due to the nature of fast-paced healthcare and residential environments
WORKING CONDITIONS
- Indoor environment including resident apartments and common areas
- Potential exposure to blood and/or bodily fluids
- Fast-paced healthcare and residential environment requiring flexibility and responsiveness
You Have:
- RN/LPN/LVN or equivalent experience with dementia care certifications preferred
- 3+ years leading memory care or specialized dementia programs
- Expertise in Alzheimer’s/dementia care, activities, and behavior management
- Knowledgeable in state and federal regulations and compliance standards governing assisted living and/or memory care
- Genuine passion for serving older adults in a memory care environment
