Director of Home Health (PACE Modesto)

The Director of Home Health Services oversees all aspects of the Home Health program and associated patient care and staff providing care in this program.  Responsibilities include regulatory and compliance oversight, patient and staffing assignments, coordination of care, assessment of patient needs and delivery of compliant care in participant’s home, skilled nursing facility, assisted living or group home and hospital discharge/continuity of care environments.  The Director is responsible for collaborating with a variety of departments and services to insure appropriate and effective care is provided to participants with continuity of care as a primary focus.

This position will work at our Modesto PACE Center, Scheduled from 8:00am – 5:00pm with flexibility to work before or after hours as needed.

Compensation: $155,000 – $175,000 Annual Base Salary

Duties and Responsibilities

• Develops, plans and implements all aspects of the Home Health Program including oversight of policy and procedure development
• Responsible for the delivery of care for all patients receiving care through the Program
• Coordinate with Interdisciplinary Teams (IDT) to insure collaboration and coordination of services and care plans
• Collaborate with both internal and external partners, services, and vendors to insure that comprehensive care is provided to patients
• Ensure compliance with all Federal, State, Regulatory, and Organizational requirements, standards, policies and practices
• Maintain effective departmental leadership and staffing to provide effective quality care for patients
• Oversees quality program and compliance initiatives and programs, including oversight of audits, care review, and plans of correction
• Provide patient assessments, treatment, and support to the care team in providing services
• Develop and oversee all fiscal aspects of program including budgeting, contract negotiation and review, insuring fiscal responsibility and viability of program services
• Provide support for staff and patients during and after hours to insure adequate care services are available
• Assist and collaborate with Senior Leadership with as it relates to Home Health Program integration and services to patients across service lines
• Other Duties as assigned

Physical Demands

• Ability to lift up to 30 pounds Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate. 
• Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
• Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
• Must have high manual dexterity.
• Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.

Work Environment

• Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
• Subject to unpleasant odors.
• Frequent travel throughout service area
• The noise level is usually quiet to moderate, but may at times be noisy and crowded.

Education/Experience Requirements    

Minimum Qualifications

• Valid CA Driver’s License, acceptable driving record, and vehicle insurance.
• Must be able to work effectively in a team environment.
• Must be able to treat geriatric patients.
• Only act within the scope of his or her authority to practice.
• Ability to communicate skillfully, handle multiple priorities, maintain a warm and friendly demeanor, perform nursing duties, be flexible, innovative, focus on results and travel regularly.
• Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
• Meet a standardized set of competencies established by Central Valley PACE and approved by CMS before working independently.

Education/Experience

• Graduation from an accredited Nurse Practitioner (NP) or Physician’s Assistant (PA-C) program
• Master’s degree healthcare administration or related field preferred.
• Current CA RN License or PAC-Certification required
• Current BLS CPR Card certified by American Heart Association.
• Minimum five (5) years’ supervisory or management experience in a health care setting.
• Minimum two (2) years home health experience.
• Minimum two (2) year experience working with the frail or elderly population.
