Compensation
- Salary Range: $109,762 – $135,000 annually
- Compensation based on years of experience

Position Summary
We are seeking an experienced and strategic healthcare leader for a Director of Case Management opportunity within an acute care hospital setting. This leadership role is responsible for overseeing all aspects of hospital case management operations, including care coordination, discharge planning, utilization management, compliance, staffing, and performance improvement initiatives.
The Director of Case Management will collaborate closely with executive leadership, physicians, nursing teams, and ancillary departments to optimize patient outcomes, improve operational efficiency, and ensure continuity of care across the healthcare continuum.
Key Responsibilities
Department Leadership & Operations
- Direct and oversee all case management department operations
- Manage staffing models, scheduling, onboarding, and competency development
- Evaluate departmental workflows, productivity, and skill mix
- Optimize utilization of technology and case management systems
- Ensure efficient resource allocation to support quality patient care
- Oversee discharge planning and care coordination processes
Performance Improvement & Compliance
- Monitor departmental metrics and operational performance
- Lead initiatives focused on improving patient flow and reducing length of stay
- Implement sustainable process improvement strategies
- Ensure compliance with organizational policies, regulatory standards, and SOPs
- Analyze data trends and identify opportunities for operational enhancement
Stakeholder Collaboration
- Collaborate with executive leadership and interdisciplinary teams including:
- Chief Medical Officer (CMO)
- Chief Nursing Officer (CNO)
- Finance leadership
- Ancillary service departments
- Ethics and compliance teams
- Legal departments
- Partner with facility and division leadership to achieve organizational goals

Required Qualifications
Education
- Bachelor’s Degree required
- Master’s Degree preferred in:
- Nursing
- Health Administration
- Business Administration
Licensure
One of the following active licenses is required:
- Registered Nurse (RN)
- Licensed Clinical Social Worker (LCSW)
- Licensed Master Social Worker (LMSW)
Experience
- Minimum 3 years of recent acute care hospital case management experience
- Minimum 2 years of leadership experience in case management
