A hospital in Nashville, TN is seeking a Director of Case Management to provide executive-level leadership over all aspects of inpatient case management operations.

The role. Reporting to facility and division leadership, the Director holds full operational accountability for the case management department, including staffing model design, skill mix, onboarding, and staff competency development. The Director ensures the integrity of care coordination and discharge planning processes — with a focus on timeliness, documentation quality, and regulatory compliance — while actively engaging interdisciplinary teams that include the CMO, CNO, ancillary service leaders, and external agencies to support continuity of care. A significant portion of the role involves analyzing operational and outcomes data to identify performance gaps and drive sustainable improvement initiatives. The Director also partners closely with the facility CFO and division case management leadership to meet established financial and clinical resource utilization goals.

What we're looking for.
  • Bachelor's degree, required
  • Master's degree in Nursing, Health Administration, or Business Administration, preferred
  • Either Registered Nurse (RN) licensure, Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW), required
  • 3+ years' of recent experience in overall acute care Hospital Case Management required
  • 2+ years' experience in case management leadership

Schedule. Full-time, days, no weekends.

Compensation. $109,762 - $135,000 per year plus $10,000 sign-on bonus
