A Director of Case Management is needed in Nashville, TN to lead the full-scope case management department at a hospital within a larger regional health system. This is a senior operational role requiring executive presence, proven change management ability, and deep subject matter expertise in acute care case management.

The role. Reporting to the Facility CFO and Division Case Management Leader, the Director holds accountability for all daily hospital case management operations, including staffing model design, skill mix, FTE allocation, onboarding, and competency oversight. The Director drives adherence to care coordination and discharge planning processes — ensuring timeliness, documentation integrity, and quality — while continuously evaluating operational performance through data analysis and implementing sustainable improvement initiatives. A core function of the role is building and maintaining strong working relationships with the CMO, CNO, ancillary service leaders, and compliance and legal partners to integrate case management seamlessly across the organization. The Director also maintains compliance with applicable policies, standard operating procedures, and external regulatory requirements.

What we're looking for.
  • Bachelor's degree required
  • Master's degree in Nursing, Health Administration, or Business Administration preferred
  • Active Registered Nurse (RN) licensure, Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) required
  • 3+ years of recent acute care hospital case management experience required
  • 2+ years of case management leadership experience required

Schedule. Full-time, days, no weekends.

Compensation. $109,762 - $135,000 per year plus $18 sign-on bonus
