Job Overview We are seeking an experienced Director of Care Management (RN) to provide strategic and operational leadership for all care management functions within an acute care environment. This leadership role oversees case management, utilization management, discharge planning, social work services, and care transition programs to improve patient outcomes, optimize resource utilization, and ensure safe, patient-centered transitions across the continuum of care.
The ideal candidate is a collaborative healthcare leader with strong acute care case management experience, proven leadership abilities, and a passion for delivering exceptional patient-centered care.
Key Responsibilities
- Provide strategic leadership for case management, utilization management, discharge planning, social work, and care transition services.
- Lead initiatives to improve patient throughput, quality outcomes, patient satisfaction, and operational efficiency.
- Develop and execute strategic, operational, financial, and quality improvement plans.
- Monitor departmental performance metrics and implement process improvements.
- Recruit, mentor, coach, and develop high-performing care management teams.
- Collaborate with physicians, executive leadership, and community partners to strengthen care coordination.
- Oversee departmental budgets, staffing, productivity, and resource utilization.
- Ensure compliance with regulatory, accreditation, and organizational standards.
- Participate in hospital committees and provide leadership on care management initiatives.
Requirements
- Active California Registered Nurse (RN) License.
- Bachelor of Science in Nursing (BSN).
- Master's degree in a health-related or science field.
- Minimum 2 years of acute care case management experience.
- Minimum 2 years of leadership or management experience overseeing acute care case management teams.
- Strong knowledge of care management, utilization management, discharge planning, and care transition models.
- Excellent leadership, communication, organizational, and problem-solving skills.
- Proficiency with Microsoft Office applications.
