QUALIFICATIONS

Requirements:

Must be 18 or older
High School diploma or equivalent
Valid driver's license and insurance
Able to provide transportation utilizing personal vehicle (Generous mileage reimbursement included)
Proof of vehicle inspection
Registration card and vehicle insurance card

This position requires you to frequently stand, walk, climb stairs, squat, and bend. Must routinely lift 20 lbs or more.

RESPONSIBILITIES

Essential Duties and Responsibilities: 

Responsibilities
Relate, interact, and communicate positively with individuals.
Support and assist individuals in enhancing skills that promote independence, community involvement, and overall quality of life.
Participate with the Program Director in completing individual assessments.
Maintain accurate records of each individual’s progress in program areas.
Assist and/or instruct individuals in activities of daily living, including:
    • Meal preparation
    • House cleaning
    • Grocery shopping
    • Laundry and general maintenance
Support self-care skills such as:
    • Bathing
    • Dressing
    • Personal hygiene
Assist with safety and pre-vocational skills when appropriate.
Document each interaction in relation to ISP outcomes.
Motivate and assist individuals in achieving goals outlined in their ISP.
Maintain a positive, flexible, and cooperative attitude with supervisors, coworkers, and the public.
Assist individuals with medical appointments.
Communicate all relevant medical information to appropriate parties (Program Supervisors, Medical Department, Supports Coordinators, and family members).
Support individuals in developing self-advocacy skills and help procure necessary services.
Meet with Program Supervisors as needed to review responsibilities and individual needs.
Attend required trainings and in-services.
