Direct Care Worker / Caregiver (42 Hours Per Week)
Location: Near Route 1 & Route 201, Pennsylvania
Schedule:
Sunday, Monday, Tuesday, Thursday, Friday
Evening Shifts:
4:00 PM – 12:00 AM OR
5:00 PM – 1:00 AM
42 hours per week

Penn Home Health Care is seeking a compassionate, reliable, and professional Direct Care Worker to provide in-home support services to an adult participant. The caregiver will assist with activities of daily living (ADLs), instrumental activities of daily living (IADLs), personal care, and mobility support while promoting independence, dignity, and safety.
Assist with personal care, including bathing, grooming, dressing, and hygiene
Provide toileting assistance and incontinence care as needed
Assist with transfers and mobility
Monitor participant safety and well-being during the shift
Prepare light meals and snacks
Perform light housekeeping related to the participant's care
Laundry and linen changes
Medication reminders (if applicable)
Assist with routine household tasks and errands as authorized
Maintain accurate documentation and EVV compliance
Report changes in condition, safety concerns, or incidents to the agency promptly
Experience providing personal care preferred
Home care, direct care, CNA, HHA, or caregiver experience strongly preferred
Ability to safely assist with transfers and mobility
Reliable transportation
Ability to work independently and follow a care plan
Must meet all Pennsylvania home care employment requirements
