POSITION SUMMARY:

Develops, organizes, implements, and evaluates instructional material to provide individuals with the knowledge and tools necessary to manage their diabetes at an optimum level. Serves as a consultant in the hospital, home or outpatient setting and as an educator for health care providers and other groups and organizations in the community.

ESSENTIAL FUNCTIONS:

  • Assesses the patient's/client's diabetic needs, helps them set goals and recommend appropriate actions to meet those goals including physical, psychological and social needs of the patient/client with diabetes.

  • Accepts referrals for physicians, nurse practitioners, nursing staff, other hospital staff, and individuals themselves, and collaborates with colleagues in planning for their teaching and needs. Receives and contracts referrals, and plans with the individual their course of education, either class or individual sessions, or both.

  • Communicates with the health care provider on client's/patient's condition, treatment, and or changes needed.

  • Continues follow-up with clients either by phone or setting up appointments with clients/patients after basic education is complete, charting on their goal achievements and/or needs.

  • Provides consultation/education for nursing, other disciplines and the community. Serves as a role model in personal areas of expertise.

  • Utilizes current research findings to support and improve practice in the Diabetes Education Center.

  • Maintains American Association of Diabetes Educators Recognition and State of Iowa Diabetes education Recognition statuses by collecting and sending in data, maintaining standards as seen by AADE and state of Iowa.

  • Complete an annual review of all diabetic education contracts.

  • Coordinates the Diabetes Education Advisory Board Meetings annually, getting agendas to members before the meeting, conducting the meetings, and recording the minutes.

  • Consults with patients regarding continuous glucose monitor testing. This includes educating patients on how to use the monitor, insertion of monitor, removal, and analyzing the information.

  • Charges and bills patients.

  • Reviews, updates and modifies diabetic education policies and procedures.

  • Determines the equipment needs of the diabetic education area.

  • Inspects and maintains all of the equipment in diabetic education area.

  • Maintains confidentiality of all pertinent client care information assuring that client rights are protected (includes both internal and external).

  • Adheres to all CRMC safety policies and procedures.

  • Adheres to all CRMC policies and procedures.

QUALIFICATIONS:

Education:

  • Required: ASN

  • Preferred: BSN

License/Registration/Certification:

  • Required: Current RN licensure in the State of Iowa

  • Diabetic Education

Required: Completion of professional diabetes management program with a minimum of 15 hours of class time.

  • Maintain current requirements for hours in diabetic education 15 hours per year.

  • Preferred: CDCES preferred

  • Preferred: ADCES membership

  • Required: BLS certification

Experience:

  • Preferred: 5+ years clinical experience

  • Preferred: Previous leadership experience

  • Preferred: 2+ years working with multidisciplinary teams in a clinical setting

  • Preferred: Demonstrated teaching/instruction ability

Knowledge/Skills/Abilities:

  • Excellent written and verbal communication skills, required.

  • Highly developed problem-solving skills.

  • Ability to adapt to flexible work schedules and frequent interruptions.

  • Ability to prioritize to meet deadlines on daily work and special projects.

  • Must be able to work independently with minimal supervision. Strong project management skills needed.

  • Ability to learn and use various community programs.
