Job Title: Diabetes Program Nurse

Position Summary:

Cooperative Health is seeking an enthusiastic, mission driven individual to fill the position of Diabetes Program Nurse. Under general supervision of the Assistant Director of Nursing, the qualified candidate will be a registered nurse who will support the clinician in the management of patients and families living with diabetes, and act as a health care consultant by providing information on the management of diabetes for patients of all ages from infancy to geriatrics. S/he shall also support the mission, vision and values of Cooperative Health, and work towards realizing its objectives through support of teams within the department.

Principal Accountabilities/Responsibilities:

  • Provide education related to diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and the techniques of self-monitoring and injection
  • Develop and implement educational programs for diabetic staff, patients, and families
  • Advise patients as to long term complications
  • Provide clinical resource and consultation to staff and clinicians.
  • Assist patients in monitoring blood glucose for the purpose of medication control and adjustment for both insulin and non-insulin dependent diabetics
  • Assess, monitor, and implement treatment plans for patients at various stages of the disease. The focus is on prevention of complications and management of stable diabetes.
  • Document assessments, interventions, and plans including recommendations and goals, progress notes, and discharge summaries.
  • Perform other related duties as required

Education & Experience:

  • Bachelors degree from an accredited nursing school preferred
  • Currently licensed to practice as a Registered Nurse in the state of South Carolina
  • Current BLS certification
  • Certified Diabetes Educator or the ability to obtain certificate within two years of employment
  • Proven leadership abilities, time management and organizational skills, and have knowledge of policies, procedures, quality assurance and compliance
  • Previous experience with electronic medical records, with knowledge of statistics, data collection and Health Informatics preferred

  • Familiar with data collection required to meet PCMH, UDS, & Hedis quality standards
  • Time management, ability to organize and prioritize tasks, flexible, detailed oriented, innovative, and dependable

Physical Demands

  • Prolonged periods sitting or standing
  • Must be able to lift up to 25 pounds.
  • Be able to sit, stand, stoop, squat for extended periods of time throughout the day.
  • Standing or walking for extended periods throughout the day.
