Diabetes Educator CDE/MS (Clinical Office

UPMC General Internal Medicine in West Mifflin, PA is hiring a casual Diabetes Educator to join their team. In this position you will work 8-10 daylight hours/week providing diabetes education to patients. This location provides free parking and some remote work flexibility.

Purpose:

The diabetes educator is skilled in diabetes education and management. This includes clinic education, group education, and management of patient phone calls (including analysis of patient blood glucose reports with subsequent changes in medication as appropriate. She/he will support the policies, procedures, and objectives of the organization and department and is able to interpret them to others.

Responsibilities:

• Gives lectures and writes articles on diabetes related topics as invited to do so.
• Completes nursing assessment and insures dietary assessment completed prior to initiation.
• Guides the patient through obtaining insurance approval with subsequent purchase.
• Maintains American Diabetes Association Recognition or obtains it for site.
• Maintains knowledge of diabetes equipment (glucose meters, insulin pumps and trains staff and patients in its use as requested.
• Demonstrates a service-oriented approach to her/his position by conveying courtesy, respect, enthusiasm and a positive attitude in work situations and public functions.
• Understands use of all insulin delivery devices and can instruct others in their use.
• Provides direct care to patients in accordance with hospital policy/procedure.
• Provides patient with educational material and video tapes of the two available pumps on the market.
• Demonstrates a professional commitment to patients and nursing staff.
• Answers patient phone calls during clinic hours, consulting with physician as appropriate.
• Keep up to date on new insulin pump models available, and new functions each offers.
• Maintains primary responsibility of coordinating patients initiation of continuous subcutaneous insulin infusion (CSII or pump therapy.
• Manages the insulin regimen on a daily basis with the patient until regulation is achieved.
• Keeps abreast of new home glucose monitoring equipment available, and how to use them.
• Participates in conferences and/or continuing education programs to further own professional growth and enhance knowledge of diabetes. Maintains CEU's necessary to maintain recognition if not CDE.
• Assists with diabetes and/or endocrine research protocols as needed.
• Explains pump therapy and answers questions of any patient interested in CSII.
• Schedules the start-up date and conducts the training.
• Orders appropriate educational material for clinic as needed.
• Educates individual clinic patients in all aspects of diabetes care.
• Minimum of 3 years of clinical experience.
• Experience in diabetes education strongly recommended.

Licensure, Certifications, and Clearances:

• CDCES (Certified Diabetes Care and Education Specialist or eligible to take CDCES exam within 3 years of hire.

• Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
