About the Role:
The Diabetes Care Director will lead and oversee comprehensive diabetes management programs aimed at improving patient outcomes across diverse healthcare settings. This role involves strategic planning, implementation, and continuous evaluation of diabetes care initiatives to ensure adherence to the latest clinical guidelines and best practices. The director will collaborate with multidisciplinary teams including physicians, nurses, dietitians, and educators to develop patient-centered care plans and educational resources. Additionally, the role requires monitoring program effectiveness through data analysis and quality improvement measures to drive enhanced care delivery.
