Hourly Pay Range: $20.00-$21.55
Shift: Tuesday-Saturday 7am-3pm
Responsible for the implementation of resident programming as required. Serves as staff leader during assigned shifts.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
• Participates as a member of the Interdisciplinary Program Team in Program Development, assessment of ILS skills, writing, including revisions to current programs as needed, and implementing the programs.
• Responsible for counting of statistics from individual program areas and completing documentation for 45 and 90 day reviews as assigned and reviewed by the Program Administrator.
• Responsible for the ILS portion of the CFA for each individual and an annual update.
• Responsible for the coordination of the annual CFA for each individual, ensuring the CFA is updated annually and most current placed in the file.
• Responsible for the Service Plan for each individual in conjunction with the site PA.
• Responsible for the purging of files (main and daily books) to ensure most current Program Plans are in place.
• Responsible for sending information to Day Programs as needed.
• Responsible for observations at the Day Program site quarterly and at the ICF as assigned.
• Use of Client Track for individual information /Plan of Care.
• In absence of Team Leader, functions as the Team Leader on shifts or as assigned. Responsible for staff training on program areas to staff as needed.
• Attends all agency mandated in-service training sessions including SCIP-R training.
• Complies with all ICF/DD and agency policies and procedures.
• Supervise residents to ensure their health, safety and welfare.
• Administers medication.
• Responsible for completion of all pertinent health related forms as needed.
• Responsible for attending staff meetings as scheduled.
• Responsible for the successful completion of all required training in a timely manner.
• Performs CPR and Strategies for Crisis Intervention and Prevention (SCIP-R).
• Complies with all agency policies and procedures.
• Other duties as assigned.
MINIMUM QUALIFICATIONS:
• High School Diploma or GED.
• At least one year of experience.
• Valid Drivers License that meets agency policy.
• Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, stooping, squatting, kneeling and climbing.
• Lifting requirement of 35 lbs.
• Ability to read and comprehend plans and documentation.
• Basic computer skills necessary for communication and documentation.
• Ability to lift and transfer individuals manually and mechanically as required by prescribed program/plan.
• Ability to move, reposition, and place individuals in pieces of equipment.
• Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelines with the developmentally disabled population.
• CPR certification required.
SUPERVISORY RESPONSIBILITIES:
This position has no supervisory responsibilities.
